   To make or change an appointment other than a 10-minute check-up appointment, you can send an e-mail message to our desk         clerk or contact her by telephone.                 

• print an appointment card

• order new elastics

• change your personal details

• view your medical history and change it if necessary

• view your photos

• view declarations
